Form Instructions:
When you click the link in the browser, your browser may Open the document, or it may prompt you to Save it to disk. If the latter, you will have to open it yourself from the location your computer stores downloaded files.

Once the form is filled out by typing in the data, click the "Submit by E-mail" button. It should ask if you want to use an email Desktop Email Application (like Outlook, etc.) or use Internet Mail (website like Gmail, Yahoo, Hotmail, AOL, etc.). If you choose Application, it should open up a new email window with the form attached and with Karen's email address in the To field. You can then send the way you normally send email.

If you choose Internet Email, it will Save the document with your typed in content to your computer. You will need to note where it is being saved, and then create an email to Karen.Moawad@gmail.com manually, and attach the document. The document will be titled 2009_Hummingbird_Needs_Assessment_Form1.pdf (your browser may append another number to the end of the filename if you have downloaded multiple times).

Special note for Apple Mac users: If your Mac opens PDF documents with the "Preview" program by default, you will need to download/install the Adobe Acrobat Reader from http://get.adobe.com/reader/ in order to save your data and Submit by E-mail.
